What is Youth Transition and PACT?

Positive Access to Community Transition

Positive Access to Community Transition (PACT) is a free weekly Daily Living Skills class offered by CFI for young adults with disabilities between the ages of 14 and 25, to assist them in reaching individual dreams, education, empowerment and independent living.

The classes cover a wide variety of subjects which the young adults themselves define as areas where they need to strengthen their life skills. A subject is covered for several weeks to assist the student in gaining a better understanding of the material.

What is Transition?

Transition is defined as a change, passage, or progression from one place, action, topic, mood to another.

For example: What will you do after high school graduation? Will you go to college? Will you get a job? What about getting an apartment? Are there services or programs that can help?
